河南省2008-2017年围产儿出生缺陷监测情况分析

Analysis of birth defects surveillance among perinatal infant in Henan Province from 2008 to 2017

  • 摘要: 目的 了解河南省2008-2017年出生缺陷发生率的变化趋势,为制定和完善出生缺陷的预防对策及干预措施提供依据。方法 对河南省2008-2017年出生缺陷医院监测数据采用SAS 9.13软件进行统计分析。结果 河南省2008-2017年共监测围产儿2615717例,出生缺陷发生率为114.11/万,各年度出生缺陷发生率呈上升趋势(χ2=9.45,P<0.001)。城市围产儿出生缺陷发生率高于农村,男性围产儿出生缺陷发生率高于女性,差异均有统计学意义(P<0.05)。产妇年龄<20岁、20岁~、25岁~、30岁~、≥35岁所出生围产儿出生缺陷发生率差异有统计学意义(P<0.05)。<20岁、≥35岁产妇更易发生围产儿出生缺陷。先天性心脏病和多指(趾)发生率呈上升趋势,总唇裂和脊柱裂发生率呈下降趋势,且先天性心脏病和脊柱裂的变化幅度较大。结论 河南省出生缺陷发生率总体有增加的趋势。随着高龄孕产妇的增加,应加强产前筛查及产前诊断工作,提高出生缺陷发现能力,降低出生缺陷发生率。

     

    Abstract: Objective To understand the change trend of the incidence rate of perinatal birth defects in Henan from 2008-2017, thus to provide scientific basis for making and perfecting the prevention and intervention measures of birth defects.Methods Surveillance data on birth defect from 2008-2017 in Henan were collected for statistic analysis by SAS 9. 13 software.Results A total of 2 615 717 perinatal infants were monitored in Henan from 2008-2017, the incidence of birth defects was 114. 11/10 000, and the incidence rate was increasing year by year (χ2= 9. 45, P<0.001). The incidence of perinatal birth defects in urban areas was higher than that in rural areas, the incidence of perinatal birth defects in boys was higher than that in girls, and the differences were statistically significant (P<0.05). There was a statistically significant difference in the incidence of birth defects among the delivery women in <20, 20-, 25-, 30-, and 35 year-old age groups (P<0.05). The delivery women in <20 and ≥35 year-old age groups were vulnerable to birth defects. The incidences of congenital heart disease and multi-finger (toe) were increasing. At the same time, the incidences of total cleft lip and spina bifida were decreasing. The variation range of congenital heart disease and spina bifida was great.Conclusion The incidence rate of birth defects shows a rising trend in Henan from 2008-2017.With the increase of older pregnant women, it is necessary to further enhance the technical level of prenatal screening and prenatal diagnosis services, improve the ability to detect birth defects, and reduce the incidence of perinatal birth defects.
