新冠疫情防控期间个案报告:基于互联网平台的妊娠期合并症多学科管理

摘要: 目的 新型冠状病毒肺炎(简称新冠肺炎)疫情期间,妊娠期合并症的孕妇管理面临极大的挑战。本团队通过基于互联网平台的线上管理模式,探索多学科进行妊娠期合并症高危孕妇的管理的可行性和有效性,总结管理诊治经过,供助产机构及健康管理人员参考。方法 在新冠肺炎防控期间,北京协和医院产科联合肾内科、营养科、心理医学科医生及药剂师为1例妊娠合并慢性肾病、肾功能不全、肾性高血压孕妇进行多学科联合会诊,制定诊疗意见,并通过互联网健康管理平台以及“安心防疫-健康管理强化服务”进行居家管理随访。结果 对患妊娠期合并症孕妇实施线上多学科管理和健康管理师配合指导随访的模式,可以良好地控制体重、血压,养成良好的健康生活和监测记录习惯,具有可行性和有效性。结论 线下和线上管理方式的结合有利于新冠疫情期间妊娠合并症的管理,即便疫情结束,此管理模式也具有推广意义。Abstract: Objective During the epidemic period of Corona Virus Disease 2019 (COVID-19),the management of pregnant women with complication is facing a great challenge. This article aimed to explore the feasibility and effectiveness of multi-disciplinary management of pregnant women with complications based on Internet platform,and the processwas summarized for the reference of midwifery institutions and health managers. Method During the epidemic period,the obstetrics department of Peking Union Medical College Hospital combined with the Department of Nephrology,Clinical Nutrition,Psychology and Pharmacy conducted a multidisciplinary consultation for a pregnant woman with chronic kidney disease,renal insufficiency and renal hypertension. Then,the patient was followed up at home through the Internet-based platform. Result Internet-based multi-disciplinary management with health managers to guide and follow up is feasible and effective for pregnant women with complications. The gestational weight gain and blood pressure were well controlled. The patient developed health lifestyle.Conclusion The combination of offline and online management is conducive to the management of pregnancy complications during the COVID-19. This model is of great significance for promotion even if the epidemic is over.
