维生素D与血钙水平对儿童骨密度的作用研究

Study on the effects of vitamin D and serum calcium levels on bone mineral density among children

  • 摘要: 目的 分析0~5岁儿童血清维生素D与总钙水平和骨密度(BMD)之间的关系,为预防和治疗佝偻病提供一定的理论参考。方法 2016年12月-2019年12月,选取就诊于江门市新会区第二人民医院和新会区妇幼保健院儿保科门诊正常体检的0~5岁儿童487名,对比不同性别、年龄、喂养方法、户外活动及维生素D补充的儿童的Z值和BMD异常情况,观察不同血清25(OH)D、血钙水平下的孩子的BMD值以及BMD异常率,用Pearson分析探究血清25(OH)D、血钙水平与BMD之间的关系。结果 487名儿童,有108名BMD异常,异常发生率为22.18%。随着儿童年龄的增加,BMD异常比率显著增加(P=0.038),随着户外活动时长的增加,BMD异常率明显降低(P<0.001)。儿童的血清25(OH)D含量升高,BMD异常率逐渐降低(P<0.001)。儿童血钙水平≥2.25 mmol/L骨密度异常率低于血钙水平<2.25 mmol的骨密度异常率(P<0.001)。Pearson相关性分析显示,Z值为-1~1时,血清中25(OH)D水平(r=0.736)和总钙水平(r=0.0.617)与Z值都是正相关(P<0.001)。结论 0~5岁儿童BMD异常情况较为严重,其Z值与血清中25(OH)D和总血钙水平表现出正相关。同时,户外活动有望减少儿童的BMD异常率。维生素D与补充性钙剂应科学合理地补充。

     

    Abstract: Objective To analyze the relationship between serum vitamin D, total calcium level and bone mineral density(BMD)among children aged 0-5 years old, and to provide a theoretical reference for the prevention and treatment of rickets.Methods From December 2016 to December 2019, 487 children aged 0-5 years old who were admitted to the Xinhui District Second People's Hospital and the Xinhui District Women's Insurance Hospital Pediatric Outpatient Department for normal physical examination were selected.The Z value and abnormal bone mineral density of children with different sex, age, feeding methods, outdoor activities and vitamin D supplementation were compared, and the abnormal rate of bone mineral density(BMD) and bone mineral density(BMD) were observed under different serum 25(OH) D and serum calcium levels. Pearson analysis was used to explore the relationship between serum 25(OH) D, serum calcium level and BMD.Results Of the 487 children, 108 had abnormal BMD, and the abnormal rate was22.18%. With the increase of children's age, the abnormal rate of BMD increased significantly(P=0.038)and the abnormal rate of BMD decreased significantly with the the increase of outdoor activities time(P<0.001). The serum 25(OH) D content increased,and the abnormal rate of bone density gradually decreased(P<0.001). The abnormal rate of bone mineral density(BMD) in children with serum calcium level ≥2.25 mmol/L is lower than that in children with serum calcium level <2.25 mmol(P<0.001). Pearson correlation analysis showed that when the Z value is-1 to 1, the serum 25(OH) D level(r= 0.736) and total calcium level(r=0.0.617) were positively correlated with the Z value(P<0.001).Conclusion The abnormality of BMD in children aged 0-5 years old is more serious. The Z values are positively correlated with serum 25(OH) D and total blood calcium levels, and outdoor activities are expected to reduce the abnormal rate of BMD in children. Vitamin D and supplemental calcium should be supplemented scientifically and reasonably.
