2011—2020年广东省广州市双胎妊娠变化趋势及围产儿结局分析

Trend and perinatal outcomes of twin births in Guangzhou, Guangdong, 2011—2020

    目的 分析2011—2020年广东省广州市双胎妊娠变化趋势及围产儿结局, 为以需求为导向的妇幼保健管理策略的动态调整提供依据。
    方法 以2011—2020年广州户籍、分娩孕周≥28周的住院分娩产妇及所生围产儿为研究对象, 回顾性分析双胎妊娠发生率变化趋势、产妇特征及围产儿结局差异。
    结果 2011—2020年广州市双胎妊娠发生率由2.79%上升至3.32%, 整体呈上升趋势(χ2趋势 = 61.624, P < 0.001);母亲年龄 < 35岁的双胎妊娠发生率相对稳定, 为2.50% ~ 3.61%;母亲年龄≥35岁高于母亲年龄 < 35岁的双胎妊娠发生率, 但整体呈下降趋势(χ2趋势 = 54.473, P < 0.001)。双胎妊娠产妇年龄从2011年的(30.21 ± 4.61)岁上升至2020年的(32.82 ± 4.80)岁, 同时, 经产妇的比例呈上升趋势(χ2趋势 = 1 300.138, P < 0.001)。2011—2020年广州市双胎妊娠活产儿的早产发生率、低出生体重发生率、低Apgar评分发生率均呈下降趋势(P < 0.001), 双胎妊娠胎儿丢失率变化差异无统计学意义(P > 0.05)。
    结论 2011—2020年广州市双胎妊娠发生率持续上升, 双胎妊娠产妇年龄增大, 经产妇比例明显升高;双胎妊娠的活产儿不良结局虽有改善, 但仍需密切关注双胎妊娠围产保健质量, 并保障母婴安全。

     

    Abstract:
    Objective To analyze the trend and perinatal outcomes of twin births in Guangzhou, Guangdong Province from 2011 to 2020, so as to provide evidence for dynamic adjustment of demand-oriented maternal and child health management strategies.
    Methods In-hospital pregnant women (≥28 weeks of gestational at delivery) with locally registered residency and their perinatal newborns were recruited as study subjects between 2011 and 2020. The trend of twin pregnancies, maternal characteristics, and perinatal outcomes were analyzed retrospectively.
    Results The incidence of twin births in Guangzhou increased from 2.79% to 3.32% with an overall upward trend (χ2trend = 61.624, P < 0.001) from 2011 to 2020. The incidence of twin births among mothers aged < 35 fluctuated between 2.50% and 3.61%. The incidence of twin births among mothers aged ≥35 was higher than that of mothers aged < 35, showing an overall downward trend(χ2trend = 54.473, P < 0.001). The average age of parturients with twin pregnancies increased from (30.21 ± 4.61) years in 2011 to (32.82 ± 4.80) years in 2020, while the percentage of multiparas was increasing(χ2trend = 1 300.138, P < 0.001). From 2011 to 2020, the incidence of preterm birth, low birth weight and low Apgar score among live twin births were decreasing(P < 0.001), and changes of fetal loss incidence were not different (P > 0.05).
    Conclusion The incidence of twin births is increasing from 2011 to 2020. Parturients with twin pregnancies are becoming older, and the percentage of multiparas is increasing significantly. Although the adverse outcomes of live twin births have been improved, quality of perinatal health care still needs to be paid close attention to ensure maternal and child safety.
