母乳吸吮对缓解足跟采血所致足月新生儿疼痛效果的meta分析

Meta-analysis of the effect of breastfeeding on pain relief caused by heel blood sampling in full term newborns

    目的 通过meta分析评价母乳吸吮对减轻新生儿足跟采血疼痛的应用效果。
    方法 检索PubMed、Embase、Cochrane Library、中国生物医学文献数据库、中国知网、万方、维普数据库中有关母乳吸吮干预减轻新生儿足跟采血疼痛的随机对照试验, 检索时间均为建库至2022年9月。采用Revman 5.4软件对检索结果进行meta分析。
    结果 通过对被采纳的10篇文献进行meta分析, 结果显示, 采血时进行母乳吸吮的新生儿, 其疼痛量表结果(WMD = - 2.21, 95% CI: - 2.34 ~ - 2.07)、新生儿心率(WMD = - 11.80, 95% CI: - 16.82 ~ - 6.78)、啼哭时间(WMD = - 39.34, 95% CI: - 47.21 ~ - 31.47)与未进行母乳吸吮的新生儿比较, 差异均具有统计学意义(均P < 0.05)。
    结论 母乳吸吮能够减轻新生儿足跟采血时的疼痛、缩短采血时的啼哭时间、降低足跟采血时的心率波动, 但受限于纳入的研究方法、研究数量及研究质量的影响, 仍需开展更多高质量、大样本和多中心的临床随机对照试验, 以进一步探索和评价母乳吸吮的应用成效, 增加证据强度。

     

    Abstract:
    Objective To evaluate the effect of breast milk sucking on pain relief caused by heel blood sampling in full-term newborns based on meta-analysis.
    Methods Randomized controlled trials on the effect of breast milk sucking on pain relief caused by heel blood sampling in full-term newborns were searched from the PubMed, EMbase, Cochrane Library, China Biology Medicine disc, CNKI, Wanfang Database and VIP database. Time of retrieval was from its inception to September 2022. RevMan 5.4 software was used for meta-analysis.
    Results 10 articles were finally involved in meta-analysis. Compared with newborns who didn't such breast milk, those who did it showed different results in terms of neonatal pain scale (WMD = - 2.21, 95% CI: - 2.34 - - 2.07), heart rate (WMD = - 11.80, 95% CI: - 16.82 - - 6.78), and time of crying (WMD = - 39.34, 95% CI: - 47.21 - - 31.47), the differences were statistically significant (all P < 0.05).
    Conclusion Breast milk sucking can relieve the pain of neonatal heel blood sampling, shorten the time of crying and reduce their heart rates. However, due to the limitation of the study methods, quantity and quality of study, more high-quality and large-scale studies are still needed for further verification.
