备孕育龄夫妇待孕时间的生活行为影响因素分析

Analysis of Lifestyle Behavior Influencing Factors on the Time-to-Pregnancy for Couples of Childbearing Age

    目的 了解接受孕前优生健康检查的备孕育龄人群待孕时间(time-to-pregnancy, TTP)的生活行为影响因素,为指导制定有效的孕前健康管理措施提供依据。
    方法  选取2022年1 — 6月广东省广州市白云区接受免费孕前优生健康检查的备孕夫妇,在其检查后的第13 ~ 15个月通过电话,调查其妊娠情况和生活行为因素,并通过多因素Cox回归模型分析TTP的影响因素。
    结果  最终随访到1684对夫妇,其中1127对(66.92%)夫妇怀孕,TTP为6个月时和12个月时的受孕率分别为49.39%和78.68%。多因素Cox风险比例回归模型分析结果显示,女性生育力较低的生活行为影响因素包括:超重/肥胖(FR = 0.79,95% CI:0.65 ~ 0.96,P = 0.018)、睡眠质量差(FR = 0.67,95% CI:0.55 ~ 0.81,P < 0.001)、配偶超重/肥胖(FR = 0.87,95% CI:0.77 ~ 0.98,P = 0.029)。女性生育力较高的生活行为影响因素包括:睡眠时间长(FR = 1.17,95% CI:1.09 ~ 1.25,P < 0.001)、每周运动频率 > 3次(FR = 1.15,95% CI:1.03 ~ 1.28,P = 0.010)。
    结论 超重/肥胖、睡眠和运动均是备孕夫妇待孕时间的影响因素,备孕夫妇的孕前保健内容中应该加强生活行为方面的健康教育和指导。

     

    Abstract:
    Objective To understand the lifestyle and behavioral factors influencing the time-to-pregnancy (TTP) among the pre-pregnancy population who received the National Free Pre-conception Health Examination.
    Methods Couples who received the National Free Pre-conception Health Examination in Baiyun District of Guangzhou from January to June 2022 were selected. At the 13th to 15th month after the examination, the pregnancy status and life behavior factors were investigated by telephone questionnaire, and the influencing factors of TTP of pre-pregnancy couples were analyzed by multivariate Cox regression model.
    Results A total of 1 684 couples completing the final follow-up. Among them, 1 127 couples (66.92%) achieved pregnancy. The conception rates were 49.39% at 6 months and 78.68% at 12 months of TTP. The results of multivariate Cox proportional hazards regression model analysis showed that the life behavior factors affecting female fertility decline included overweight/obesity (FR = 0.79, 95% CI: 0.65 − 0.96, P = 0.018), poor sleep quality (FR = 0.67, 95% CI: 0.55 − 0.81, P < 0.001), overweight/obesity in spouse (FR = 0.87, 95% CI: 0.77 − 0.98, P = 0.029).The lifestyle factors influencing the improvement of female fertility included long sleep duration (FR = 1.17, 95% CI: 1.09 − 1.25, P < 0.001) and exercising more than 3 times per week (FR = 1.15, 95% CI: 1.03 − 1.28, P = 0.010).
    Conclusions Overweight/obesity, sleep and exercise are the influencing factors of the time to pregnancy. Health education and guidance on life behavior should be strengthened in the content of pre-pregnancy health care for couples preparing for pregnancy.
