会阴超声观察不同分娩方式产后女性盆腔器官位置及移动度的变化

Observation on the change of positions and movements of postpartum female pelvic organs under different delivery ways by perineum ultrasound

  • 摘要: 目的 应用会阴超声观察不同分娩方式女性盆腔器官位置及移动度的变化。方法 选取2015年1月-2016年7月深圳市龙岗区妇幼保健院产科分娩后6~8周初产妇100例,依照不同分娩方式分阴道分娩组52例、选择性剖宫产组48例;未育对照组50例,观察不同分娩组女性和对照组分别在静息和瓦氏动作时盆腔器官位置及移动度变化,各参数对比分析。结果 (1)静息、瓦氏动作时,阴道分娩组UVJ-VD、CV-VD位置低于剖宫产组UVJ-VD、CV-VD,差异有统计学意义(P<0.05);静息、瓦氏动作时阴道分娩组和剖宫产组的UVJ-VD、CV-VD、ARJ-VD均低于对照组,差异有统计学意义(P<0.05)。②阴道分娩组、剖宫产组UVJ、CV及ARJ移动度均大于对照组,差异有统计学意义(P<0.05)。③阴道分娩组和剖宫产组的前、中、后盆腔脱垂率均高于对照组,差异有统计学意义(P<0.05);阴道分娩组前盆腔脱垂率高于剖宫产组,差异有统计学意义(P<0.05);中、后盆腔脱垂率阴道分娩组稍高于剖宫产组,但二者的差异无统计学意义(P>0.05)。结论 不同分娩方式会对女性盆腔器官的位置及移动度造成不同程度的影响。

     

    Abstract: Objective To observe the changes of positions and movements of female pelvic organs after delivery under different delivery ways by perineum ultrasound.Methods A total of 100 primiparas who gave birth after 6-8 weeks in our hospital from January of 2015 to July of 2016 were recruited. According to the delivery way, 52 cases were divided into vaginal delivery group and 48 cases was divided in elective cesarean section group. And 50 women who never pregnant were as contrast group. The changes of pelvic organ positions and movements at the statement of rest and Valsalva maneuver in different groups were observed. The parameters were compared and analyzed.Results ① At the statement of rest and Valsalva maneuver, the UVJ-VD、CV-VD of vaginal delivery group were lower than those of cesarean section group. The difference between the two groups was statistically significant (P<0.05). At the statement of rest and Valsalva maneuver, The UVJ-VD, CV-VD and ARJ-VD in vaginal delivery group and the cesarean section group were lower than those in the control group. The difference between the two groups was statistically significant (P<0.05).② The degree of movement of UVJ, CV and ARJ in the vaginal delivery group and cesarean section group were all greater than those in the control group. The difference was statistically significant (P<0.05). ③The rates of anterior, middle and posterior pelvic prolapse in vaginal delivery group and the cesarean section group were all higher than those in the control group. The difference was statistically significant (P<0.05). The rate of anterior pelvic prolapse in vaginal delivery group was higher than that in the cesarean section group. The difference between the two groups was statistically significant (P<0.05). The rates of middle and posterior pelvic prolapse in vaginal delivery group were slight higher than those in the cesarean section group. However, there were no significant difference between the two groups (P> 0.05).Conclusion Different delivery ways have different impacts on the positons and movements of female pelvic organs.
