Abstract:
Objective To analyze the diagnostic value of prenatal ultrasound screening in the diagnosis of fetal malformation during second trimester.
Methods At total of 4560 cases of pregnant women who received prenatal ultrasound examination and were diagnosed as single pregnancy in our hospital from June, 2017 to June, 2016 were selected as the research subjects. The accuracy, sensitivity and specificity of prenatal ultrasound examination for fetal malformation during second trimester were statistically analyzed.The gold standard was the result of delivery and induced labor. The application value of prenatal ultrasound screening in the diagnosis of fetal malformation during second trimester was to be explored.
Results Among the 4560 fetuses, 61 cases of fetal malformation were confirmed after birth or induced labor. They included 20 cases of cardiac abnormalities, 9 cases of renal dysplasia, 8 cases of cleft lip and palate, 3 cases of pulmonary cystadenoma, 4 cases of skeletal abnormalities, 3 cases of varus, 2 cases of diaphragmatic hernia, 2 cases of pulmonary sequestration, 3 cases of 2 cases of spina bifida, 2 cases of celiac cysts, 2 cases of edema, 1 cases of agenesis of the corpus callosum and 2 cases of multi finger malformation. The incidence of fetal malformation was 1.21%. Among the 4560 fetuses, 55 cases of fetal malformation were diagnosed by prenatal ultrasound examination. They included 18 cases of cardiac abnormalities, 9 cases of renal dysplasia, 7 cases of cleft lip and palate, 3 cases of pulmonary cystadenoma, 4 cases of skeletal abnormalities, 3 cases of varus, 2 cases of diaphragmatic hernia, 2 cases of pulmonary sequestration, 2 cases of spina bifida, 2 cases of abdominal cysts, 2 cases of edema and 1 cases of the absence of corpus callosum. The detection rate of fetal malformation was 1.21%. There was no statistical difference between the results of prenatal ultrasound examination and gold standard (
P> 0.05). The accuracy of prenatal ultrasonographic diagnosis of fetal malformation was 99.4%, the sensitivity was 73.8% and the specificity was 99.8%.
Conclusion Prenatal ultrasound examination can screening many kinds of fetal malformation during second trimester and has a high detection rate of fetal malformation. It can reduce the missed diagnosis and has important clinical value.
