降钙素原、C反应蛋白对1~3月龄发热患儿的临床意义

Clinical significance of procalcitonin and CRP on the diagnosis of children's fever aged 1-3 months

  • 摘要: 目的 分析降钙素原(PCT)和C反应蛋白(CRP)在1~3月龄发热患儿诊断中的意义。方法 以2016年1月-2017年1月在深圳市妇幼保健院就诊的1~3月龄发热患儿135例为研究对象,根据最终诊断结果将其分成细菌性感染组和病毒感染组,同期选择正常体检儿童60例作为对照组。分别测定3组患儿体内的PCT、CRP和血常规白细胞计数(WBC)的含量。结果 与病毒感染组发热患儿相比,细菌性感染组患儿血清中的PCT、CRP和WBC的含量明显偏高;细菌性感染组发热患儿的血清PCT和CRP的含量大多为阳性,其阳性率为93.65%和84.13%,明显高于WBC的阳性率(73.02%)。PCT和CRP作为判定标准的特异性为88.89%(64/72)和54.17%(39/72),两者之间差异有统计学意义(χ2=21.312,P <0.01);WBC的特异性为47.22%(34/72),明显低于PCT和CRP的特异性。结论 测定1~3月龄发热患儿血清中PCT和CRP的含量有助于快速确定发热患儿的病因,适合临床应用。

     

    Abstract: Objective To analyze the significance of procalcitonin (PCT) and C reactive protein (CRP) in the diagnosis of children's fever aged 1-3 months.Methods From January 2016 to January 2017, 135 children aged 1-3 months with fever who treated in maternal and child care hospital of Shenzhen were selected as objects, and they were divided into bacterial infection group and virus infection group according the final diagnosis. At the same period, 60 normal physical examination children were selected as control group. The contents of PCT, CRP and routine blood count (WBC) were measured among of the children.Results Compared with viral infection group, the levels of PCT, CRP and WBC in the serum of the children in bacterial infection group were higher obviously. The serum levels of PCT and CRP were positive in children with fever in bacterial infection group and the positive rates were 93. 65% and 84. 13%, which was higher than that of WBC (73. 02%). The specificity of PCT and CRP as criterion were 88. 89%and 54. 17% respectively, and the difference was statistically significant (χ2=21. 312, P< 0. 01). The specificity of WBC was 47. 22% (34/72), which was lower than the specificities of PCT and CRP.Conclusion The determination of PCT and CRP in serum in children aged 1-3 months with fever is conductive to rapidly determine the cause of fever and is suitable for clinical application.
