多囊卵巢综合征妊娠患者的产科结局及其影响因素

Obstetric outcome and its influencing factors in pregnant women with polycystic ovary syndrome

  • 摘要: 目的 调查多囊卵巢综合征妊娠患者的产科结局,分析影响其产科结局的相关因素。方法 采用问卷调查法分析深圳市人民医院2014年1月-2016年12月收治的149例多囊卵巢综合征妊娠患者的一般资料(年龄、体重指数、腰臀比、妊娠期合并症、激素水平等)及产科结局,采用单因素及Logistic回归多因素分析影响多囊卵巢综合征妊娠患者产科结局的相关因素。结果 149例多囊卵巢综合征妊娠患者,发生不良妊娠结局52例,发生率为34.90%,其中自然流产34例、早产9例、低出生体重6例、新生儿窒息3例。体重指数、腰臀比、妊娠期糖尿病、睾酮≥ 45μg/L是导致多囊卵巢综合征妊娠患者发生不良妊娠结局的影响因素(P<0.05),而年龄、不孕年限、妊娠期高血压、促性腺激素使用总量及黄体生成素<5μg/L对多囊卵巢综合征妊娠患者产科结局无显著影响(P>0.05)。结论 多囊卵巢综合征妊娠患者不良妊娠结局发生率较高,临床应加强其危险因素监测评估,以便及时有效采取干预措施,改善母婴安全。

     

    Abstract: Objective To investigate the obstetric outcomes of pregnant women with polycystic ovary syndrome (PCOS), and to analyze the related factors that affect the obstetric outcomes. Method Questionnaire method was used among 149 cases of pregnant women with polycystic ovary syndrome in our hospital from January 2014 to December 2016, and the information of age, body mass index, waist-to hip ratio, pregnancy complications, hormone level and the obstetric outcomes were collected and analyzed by using univariate and multivariate logistic regression.Results Among the 149 pregnant women with polycystic ovary syndrome, the incidence of adverse pregnancy outcomes was 34. 90% (52 cases), which including 34 spontaneous abortion, 9 premature delivery, 6 low birth weight and 3 neonatal asphyxia. The influencing factors of adverse pregnancy outcomes in pregnant women with polycystic ovary syndrome were body mass index, waist hip ratio, gestational diabetes and neo-hombreol F ≥ 45μg/L led to (P< 0. 05). However, age, infertility years, pregnancy hypertension and total gonadotropin using and luteinizing hormone < 5μg/L were not effecting the outcome of pregnancy in patients with polycystic ovary syndrome (P> 0. 05).Conclusion The incidence of adverse pregnancy outcomes in patients with polycystic ovary syndrome (PCOS) is high, so it is necessary to strengthen the monitoring and evaluation of risk factors, in order to adopt intervention method timely and improve women and children's health.
