普宁市妊娠期妇女心理健康状态调查及相关因素分析

Investigation on mental health and analysis of related factors of pregnant women in Puning city

  • 摘要: 目的 分析探讨普宁市妊娠期妇女心理健康状态及相关因素。方法 选取2014年9月-2016年12月普宁市妇幼保健院计划生育服务中心和普宁市洪阳医院妇产科收治的467名妊娠期妇女为妊娠期妇女组;另外选取345名非妊娠期妇女为非妊娠期妇女组。两组妇女进行症状自评量表(SCL-90)测评,分析其心理健康状态。然后根据SCL-90测评结果,将妊娠期妇女组分为心理健康组(324例)和心理障碍组(143例)。两组妇女进行调查问卷,采用Logistic多因素检验,分析妊娠期妇女心理健康的影响因素。结果 非妊娠期妇女组和妊娠期妇女组的心理健康状态,强迫症状、偏执项目评分相近,经比较差异无统计学意义(P>0.05);但妊娠期妇女组躯体化、人际关系、抑郁、焦虑、敌对、恐惧、精神病性等项目评分高于非妊娠妇女组,差异有统计学意义(P<0.05);经单因素分析和Logistic多因素分析显示,妊娠次数 ≥ 2次和无妊娠合并症是妊娠期妇女心理健康的保护因素(OR=0.550、0.340,P<0.05),意外妊娠、与公婆关系及孕周 ≥ 30周是妊娠期妇女心理健康的危险因素(OR=3.296、1.520、2.003,P<0.05)。结论 受妊娠次数、妊娠意愿、家庭关系、生理状态和孕周等多种因素的影响,妊娠期晚期女性容易产生心理健康障碍。因此,应得到更多的关注,给予积极有效的心理干预,为孕妇营造良好的妊娠氛围。

     

    Abstract: Objective To analyze the mental health and related factors of women during pregnancy in Puning City.Methods A total of 467 pregnant women admitted to Gynecology and Obstetrics Department of Hongyang Hospital between January 2014 and December 2016 were selected as pregnant women group, and at the same time, 345 non-pregnant women were selected as the nonpregnant women group. The SCL-90 test was used to analyze the mental health status of the two groups women. According to the evaluation results of SCL-90, the pregnant women were divided into mental health group (324) and psychological disorder group (143). The two groups women were surveyed by questionnaire. Logistic multivariate analysis was used to analyze the influencing factors of mental health of pregnant women.Results The scores of mental health status, obsessive-compulsive symptoms and paranoid items of non-pregnant women and pregnant women were similar. There was no statistical significance (P> 0.05). However, somatization, interpersonal relationship, depression, anxiety, hostility, fear, paranoia and psychoticism of pregnant womenwere higher than those of non-pregnant women. The difference between the two groups was statistically significant (P< 0.05). Univariate analysis and Logistic multivariate analysis showed that the number of pregnancy times 2 or more and pregnancy-free complications were the protective factors of mental health of pregnant women (OR=0.550, 0.340, P< 0.05). Unexpected pregnancy and relationships with parents-in-law and gestational age ≥ 30 weeks were risk factors of mental health of pregnant women (OR=3.296, 1.520, 2.003, P< 0.05).Conclusion Females are susceptible to mental health problems during the third trimester of pregnancy due to multiple factors such as the number of pregnancies, pregnancy intentions, family relationships, physical conditions, and gestational age. It is necessary to pay more attention on, give positive and effective psychological intervention to, and create a good pregnancy atmosphere for pregnant women.
